How do you insert an image into an opt-in form?

Just created my first opt-in form. Trying to 'insert' an image...nothing happens. Shouldn't there be an upload button?

I have the same problem...where do you find this opt-in code. The link you gave describes how to design a form...but where is the HTML Opt in page code?
