Header Opt-in Form Display Location Options Don't Work

I've tried every possible combination of display location options for the Header Opt-in Form and they simply don't work. Either the header form shows up on every single page on my site, or it doesn't show up on any pages at all. Is this a known issue?

More specifically, choosing any of the Page location options don't seem to have any effect at all. Choosing any of the post category location options makes the header form show up on every single page across the site.

I'm on Wordpress 3.4.2 and tried it with w3 total cache disabled and enabled - both had the exact same results.

I'd love it if someone could help me fix this issue!
