Preview Not Showing Fields

My custom form is not showing the name, email, submit fields. I've followed your tutorials for integration with MailChimp and triple checked my work.

Can someone please help me?

I actually got the fields to show but they are too far to the right.

I'm trying to get my opt-in form to look like this:
The attachment E-Book_Template.jpg is no longer available
And am having a difficult time.

Any help you could provide would be most appreciated.

because you want a special design, you need to add css or to simplify your form. you don't have css skills ?
