WPSubscribes Breaks WordPress Theme

When I activate the WPSubscribers plugin it breaks my site/theme. The WordPress Admin Bar disappears, my side bar widgets disappear, and my embedded images do not render.
I've deactivated W3 Total Cache and the problem remains. When I deactivate WPSubscribers everything works fine. See the working site at http://www.thetreetopper.com/2012/11/de ... ty-lights/ and the attached image of when it doesn't work.
