I have spent the whole afternoon working with MAXCDN to find out why it doesn't work and finally it was solved by disabling WPS This is causing an issue and I have seen other posts related to this also with W3Tc

Has there been any fix and if not unfortunately we'd like a refund as the cach is the most important plugin of the site

Following my posting I find the issue is with W3 Total Cache. I de activated this plugin and installed WP Super Cache. This was easier to set up with MAX CDN and even better; work with WP Subscribers without issues! So you could call this solved
