WPSubscribers Form Based on Author ID

Trying to get a form to be on a author page and post page based on the Author ID.
On a multi author website this seems to be something more logical than basing it on category.... since Authors who want to be contributing to build up their own lists may post n different categories.
Is there anyone done this implementation before in terms of having forms based on the author of a post?
