Im sorry, im new on this and its a dumb question but where and how can i find ''your html form code from your email company'' ?

I dont know what to put on ''Enter your html opt-in code:'' box.

It's the code from Mailchimp, aweber, ... the email company you use to create your mailing list

I have the same problem...where do you find this opt-in code. The link you gave describes how to design a form...but where is the HTML Opt in page code?

It seems Mailchimp change their website,
you can find the optin code here :

Lists (choose your list) > Signup Forms > Embedded Forms
