Custom form not showing on chrome/ie

Hi, I can get the RHS sidebar form to show on rehabguide.co.uk using firefox, but it doesn't appear when using either Chrome or IE....any thoughts on how to fix this? Thanks

Hi,
I didn't see the form on all browser, can you check in the admin, if you don't have a button red with "turn to visible for all"
If you see the form only on firefox and you are connected on the admin WP and if there is the red button it's normal, just click on the red button it will become green and normally it will be ok

That fixed it, many thanks, don't know how I missed that. Ca tout marche bien maintenant
