Hi,
Perhaps it is a problem on the "detection code".
What do you have on "Form EMAIL field:" in "Setup Opt-in Code" ?
Is it "MERGE0" ?

if you had provided a direct access to you by live messaging or a temp admin access to your wordpress, we could look for it.. but for now you don't provide a lot of details, and we can't go further

Is there any resolution to this yet? I too have Mailchimp and experiencing the same problem

I found the solution in another post so I'm sharing in case anyone comes across this. I watch this video (even though I do not speak French):
http://www.youtube.com/watch?v=C7IB1NCg-G0
In short, I made 2 mods:
1. When I chose the "classic" form, I selected the options drop down to include ALL fields instead of just required fields.
2. After I selected "detect form", I didn't change anything. There are some instructions that indicate to change the URL to "/wp-content/plugins/wpsubscribers/mailchimp.php" but I left it as-is.
Now, the form works.
